Welcome — and congratulations on your acceptance. This form completes your enrollment for the 2026–2027 school year. It gives us the deeper information we need to finalize your athlete's placement, and captures your consent, waiver, and financial agreement. As an accepted student, the $99 intake fee is waived. A non-refundable $750 administrative fee is required to hold your spot.

The Athlete School is committed to protecting the personal information of all enrolled students and their families. By completing this form, you acknowledge and agree to the following:

  1. Information Use: Personal, academic, and medical information provided will be used solely for enrollment, student records, and program delivery.
  2. Confidentiality: All information will be kept confidential and not shared with third parties without express consent, except as required by law.
  3. Data Security: The Athlete School employs appropriate physical, electronic, and administrative procedures to safeguard information. No method of internet transmission is 100% secure.
  4. Access & Corrections: You have the right to access, correct, or update your personal information by contacting our administration office.
  5. Consent: Completing and submitting this form constitutes consent to the collection, use, and storage of the information provided.
Part 2 — 2026–2027 Consent, Waiver & Financial Agreement

Please read and acknowledge each section. This agreement covers consent, liability, medical authorization, financial commitment, and school policies for the full 2026–2027 school year.

Media Release

I grant permission for images or video of my child captured during school activities to be used by The Athlete School for educational, training, promotional, marketing, or social media purposes. I waive any rights to compensation or ownership of these materials.

Safety and Security

I authorize The Athlete School staff to inspect my child's belongings, including backpacks, sports bags, or personal items, if deemed necessary for safety, security, compliance with school policy, or protection of students and staff.

Transportation Permission

I permit my child to be transported by The Athlete School staff or approved adults for official school purposes, including activities, field trips, athletic training, competitions, enrichment opportunities, and school-related events. Drivers must be 25 years or older with an approved driving record.

Student Responsibility and Damages

I accept full financial responsibility for any damages caused by my child. If early return from an offsite event, trip, activity, or competition is required for disciplinary reasons, I understand that I am responsible for arranging and funding that transportation.

Acknowledgment of Risk and Liability Release

I recognize the potential risks associated with academic, athletic, enrichment, training, field trip, competition, travel, and school-related activities. These risks may include injury, illness, loss, or damage. I assume full responsibility for any injury, loss, or damage and release and hold harmless The Athlete School, its staff, owners, representatives, affiliates, and related organizations from liability for such incidents to the fullest extent permitted by law.

Medical Authorization

In the event of illness or injury, I authorize The Athlete School to arrange necessary medical treatment, including transportation to a medical facility, administration of emergency care, or consent for medical procedures as advised by a licensed healthcare provider. I accept full financial responsibility for all medical expenses incurred.

Medication Policy

All prescription medications must be in the original pharmacy-labeled container. Over-the-counter medications must be in original manufacturer packaging. Inhalers and EpiPens must have a pharmacy label if not in original packaging. Medications must be turned in at check-in for trips or kept by the student only with parent approval and school acknowledgment. No vitamins, supplements, or alternative medicines will be administered without physician documentation. School staff may provide basic first aid supplies and over-the-counter items such as acetaminophen, ibuprofen, or topical creams if permitted by the parent.

Duration of Authorization

This authorization begins on August 1, 2026 and remains valid until July 1, 2027, or until revoked in writing by the parent or guardian.

Financial Commitment Term

Enrollment in The Athlete School is a financial commitment for the 2026–2027 school year. For students beginning at the start of the school year, the commitment runs from August 2026 through May 2027. If a student enrolls mid-year, the financial commitment applies to the remaining months from the student's official start month through May 2027.

Administrative Fee & Tuition

The $750 administrative fee is required to officially hold the student's enrollment spot, and supports scheduling, staffing, and administrative preparation. This fee is non-refundable and is not a deposit or tuition credit. It will not be applied toward monthly tuition.

Auto-Draft Authorization

I authorize The Athlete School to automatically charge my approved payment method on the 1st of each month for the applicable 2026–2027 tuition period. For students starting at the beginning of the year, auto-drafts run August 2026 through May 2027. For mid-year starts, auto-drafts apply from the official start month through May 2027. A $25 late fee may be assessed for failed or late drafts.

Refund & Withdrawal Policy

Withdrawals before August 1, 2026 may receive a refund of tuition paid, excluding the non-refundable $750 administrative fee. Withdrawals after August 1, 2026 are not eligible for refunds of tuition paid or future installments. Families are responsible for the full applicable tuition commitment based on the student's enrollment period.

Parent-Athlete Code of Conduct

Parents agree to support coaches and school staff respectfully, communicate constructively, uphold team culture, and ensure the student-athlete attends and participates responsibly. Athletes agree to demonstrate consistent effort, sportsmanship, accountability, and respect for peers and coaches, and to represent The Athlete School with pride and discipline on and off the field. Violation of this code may result in dismissal from the program without refund.

Non-Payment Policy

If an account becomes more than 30 days past due, the student may be temporarily suspended from program participation until the balance is paid in full.

Final Payment Responsibility

I understand that early withdrawal does not release me from my financial obligation. I am responsible for the full applicable tuition commitment for the 2026–2027 school year, whether the student enrolls for the full year or starts mid-year and is responsible for the remaining months through May 2027.

Agreement

By signing below, I affirm that I have legal authority over the child listed above, understand and accept the risks involved in participation, agree to all terms stated in this document, understand my financial responsibility for the applicable enrollment period, and have read and agree to The Athlete School's policies and guidelines.
